APPLICATION FOR ADVERTISEMENT AND SALE PERMIT

*I/We, the undersigned hereby apply for an advertisement and sale permit
in accordance with the provisions of the Housing Development (Control and
Licensing) Regulations 1989, and for the purpose of complying with such
provisions, *I/we submit the following particulars:
1. (a) a copy of the approved building plan;
(b) three copies of each type of the proposed advertisement in
national language and the language that is being proposed if the
advertisement is as defined in regulation 2;
(bb) three copies of the proposed advertisement in national
language and the language that is being proposed if the advertisement will
be in audio or visual form;
(c) a copy of an agreement to the effect that the land-owner(s)
agree to abide by regulation 10 of the Housing Development (Control and
Licensing) Regulations 1989 (if applicable);
(d) a copy of the valuation report by a licensed valuer and
particulars of the charge (if the land is charged to a person, body of
persons, company or society other than a bank or a finance company
licensed under the Banking and Financial Institutions Act 1989 or the
Islamic Banking Act 1983, the Bank Simpanan Nasional Berhad Act 1997, the
Bank Kerjasama Rakyat Malaysia Berhad Act 1978 or the Bank Pertanian
Malaysia Act 1969.

The following particulars must be shown in
each of the proposed advertisements -
(a) the licence number of the housing developer and the
validity period;
(b) the permit number of the advertisement and sale permit
and the validity period;
(c) the name, address and telephone number of the licensed
housing developer;
(d) the tenure of the land if the land is leasehold, its
expiry date and encumbrances (if any);
(e) the building description of the proposed housing
accommodation as specified under the Fourth Schedule of Schedule G and
the Fourth Schedule of Schedule H;
(f) the name of the housing development (if any),
(g) the expected date of completion of the proposed housing
development;
(h) the selling price of each type of housing accommodation;
(i) the number of units of each type of housing accomodation;
(j) the name of the Appropriate Authority approving the
building plans and the reference number; and
(k) the name of the authorised agent , power of attorney
holder or project management company (if any).
